Simplot Release Forms

 For the athletes running for American Fork at the Simplot Games next week, you must sign your waiver before Saturday night 5 PM.

To sign release forms, have your athletes go to https://registration.simplotgames.com/sign-release-form and enter the team number (424), their first and last name then sign release form(s) by athlete and parent/guardian if under 18.
